All children aged between 5yrs and 14yrs must be accompanied by an adult during sessions. Older children may wish to have their sessions without a parent/guardian, therefore, if they are deemed ‘Gillick competent’ the parent/guardian can sit in the waiting area for the duration of the session. I ask that during the child’s session, the parent/guardian remains within the building.
By taking part you agree that you are fully committed and engaged in the process. You agree that the therapist is not responsible if you chose not to follow advice given within the sessions. Results are variable and not guaranteed.
All fees will be paid at the end of each session. Fees will be paid via bank transfer. If you need to cancel, please give 24 hours’ notice to avoid paying for session in full. For those attending Smoking Cessation and wish to cancel, please give 5 working days’ notice to receive the return of your deposit. If you are running late, please alert me as quickly as possible. I will do my very best to provide you with a full session but if there are appointments booked after yours, these must be honoured. If you are unable to complete the full session due to arriving late, payment must still be made in full.
The records I keep for all clients include the personal details you share with me such as your name and phone number etc… and brief notes which I record during sessions. These records are kept in paper and electronic form and stored and protected with reasonable security safeguarding against disclosure, loss, theft, unauthorised access, copying, use, or modification.
Disclosure of personal information will occur only in the following situations:
- If you reveal that you intend to harm yourself or another person.
- If you reveal that a child is being abused or is in danger of abuse.
- If I receive a court order to reveal your personal information
I am not authorised to give any medical advice. This advice should be sought from your GP or relevant health professional.
Your session will be immediately terminated, without refund, should it be evident that you are under the influence of alcohol or drugs.
